Students should only complete these steps once they have been fully accepted to the program.


____After acceptance, the Academy counselor will call your high school counselor for verification of graduation requirements. Then you will be called to determine a schedule of UWG classes

____Once you are registered: Contact your home high school counselor (if you are a student in a public high school in the State of Georgia) for the HOPE Accel Application. This can be downloaded here. Students and parents must complete and sign Part I (pages 3, 4, and 5), and the home high school counselor must complete Part II (page 6). Return both parts to The Academy counselor either by mail or fax it to 678-839-0636. Do not complete the on-line application. We must have the paper copy.

____Make sure immunizations are complete, including Hepatitis B (series of 3 shots), and chicken pox vaccine or physician's verification that student has had chicken pox by calling Health Services (678-839-6452). If necessary, mail or fax documentation to: Health Services, UWG, Carrollton, GA 30118, fax: 678-839-0656.

____Complete the Free Application for Federal Student Aid (FAFSA) on-line if you are applying for need-based scholarships. Answer "No" to question 25 regarding receiving a high school diploma. The results will indicate that your student is not eligible for Federal aid, but the Expected Family Contribution generated will assist Ms. Colgate in allocating internal Advanced Academy scholarships.

____Send a copy of your current 1040 or 1040EZ to Ms. Colgate if you are applying for need-based scholarships: Honors House, UWG, Carrollton, GA 30118.

____Final high school transcript showing Spring semester grades should be sent by July 1 to the Advanced Academy counselor.


____ Print off the housing contract and fill it out completely. Please make sure there is a parental signature in the upper portion of the contract. Mail the completed contract and $250 housing deposit to the address provided on the contract. Make checks payable to UWG.

____ Please follow the link below and fill out the Myers-Briggs Personality Type Indicator online. Write down the four letter personality type given to you for use later in the Roommate Questionnaire .

____Fill out the Roommate Questionnaire and submit it.

____ If you have not already reserved a spot at an upcoming Academy Orientation Session, please reserve one here.

____ Print out a campus map with parking options circled for your orientation visit.

____ Complete both the Student Entry Survey and the Parent Entry Survey online or complete them on paper (Student Entry Survey / Parent Entry Survey)

____ Print out a copy of the Academy Move-in Day Schedule

____ Fill out these Academy forms* and bring them completed to Orientation: Consent For Travel Personal Information Form Student Agreement Form Statement of Health, Permission to Treat & Statement of Financial Responsibility Release and Waiver of Liability Weekend Travel Permission Form Gunn Hall Interest Survey FERPA Waiver (We can notarize this at the orientation session) Honor Code Pledge UWG Photo Release Also, bring a copy of the front and back of the Student's Medical Insurance Card *Please note, students cannot move into Gunn Hall without completing these forms.
